To use this template in a story, copy and paste the following code into to the top of the story and fill in with the correct strings and integers. The result will look like the template to the right.

Steps to take:

1. Replace the second "Title" with the title of the work
2. Replace the second "Author" with the author of the work
3. Replace "Month" with the month (long name) and "YYYY" with the four-digit year, corresponding to when it was written or when the last major addition was made
4. Replace the second "Length" with the length of the work, using Very Short/Short/Medium/Long/Very Long
5. Replace the first # with the estimated kilobytes text (1kb is about half a page)
6. Replace "Genre or Category" with appropriate genre
7. Replace all instances of "Teen" with the rating, using Teen/Children/PG13/Adult
